Change to Aged Care

From 1 November 2025, the Home Care Package (HCP) program transitioned to a new initiative called Support at Home. This change brings updated benefits, services, and funding options aimed at enhancing in-home care for older Australians.  

CAPAH became an Approved Aged Care Provider in 2019 and is recognised as a quality aged care services provider. We currently offer the Support at Home Program, the Commonwealth Home Support Program (CHSP), Social Support Group Program, Social Support Individual Program, Community Visitors Scheme (CVS), and a Dementia Care Program.

Support at Home 

The new Support at Home program is designed for those with higher levels of need. Under Support at Home, you have access to a defined list of services to support you to live at home for longer. This can include clinical supports, such as nursing care, occupational therapy, and podiatry; independence, such as help with showering, taking medications, transport, and respite care; and everyday living, such as cleaning, gardening, shopping, or meal preparation. Commonwealth Home Support

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Program (CHSP) is a government funded program and provides for the delivery of low-level home support to older people for a short term or ongoing basis.The aim of the program is to help older people live as independently as possible — with a focus on working with you, rather than doing for you. It is about building on your strengths and abilities to help you remain living independently and safely at home. View CAPAH CHSP Prices.

Social Support - Group Program

Our Social Support Group program is designed to provide an opportunity for social interaction and support people in strengthening skills needed for daily living to maintain their confidence and independence, while participating in social support group activities. Our activities include social interaction, physical activity and mental stimulation in a safe and secure environment. 
Activities offered include music, singing and dancing; craft; restaurant lunches; special celebrations; day outings; information & education sessions; table games; and gentle exercise.

 

Social Support - Individual Program

Our social support individual programs enhance the quality of life for individuals living independently at home by providing access to community and recreational activities. 
Service is delivered by trained staff or volunteers who speak a range of languages to meet the cultural needs of our community. We can assist with shopping, bill paying and banking; accompanying to appointments and activities, monitoring via phone service; transport to appointments and activities.

Dementia Care Program

CAPAH provides assistance to consumers and support to families to help them manage the symptoms of dementia and memory loss.
Our team can provide a range of services that include: dementia specific activities; experienced supportive staff; nutritional meals; ongoing care review and support; support for family; dementia and behavioural strategy plan provided by a Registered Nurse.

Community Visitor Scheme (CVS)

The CVS program aims to enrich the quality of life of aged care home residents who are socially isolated or lonely, and would benefit from a friendly visitor. It also helps to establish links between people living in aged care homes and their local community. 
A volunteer Community Visitor is matched with a resident of an aged care home and visits them at least once a fortnight. Visits are spent on an activity both enjoy such as reading, listening to music, chatting and reminiscing, watching a favourite TV program together, taking a walk together or going on an outing.
